Rising Fist Martial Arts — Liability Waiver and Release I, the undersigned, on behalf of myself and the student named in this enrollment, acknowledge that participation in martial arts training carries inherent risks of injury. I voluntarily assume those risks and release Rising Fist Martial Arts, its instructors, owners, employees, and agents from any liability for injury, loss, or damage arising out of participation, except in cases of gross negligence or willful misconduct. I confirm the student is in good health and able to participate in physical activity, or has cleared participation with a physician. I authorize Rising Fist Martial Arts to administer or seek emergency medical care if needed. I have read this waiver, understand it, and agree to its terms.
